Waiver/Release & Media Consent* This field is required.I certify that the students I am registering are in good health and have no physical, mental, or emotional problems that are likely to prevent participation in strenuous physical play in any sporting activities. I assume full responsibility and agree to hereby release Jason Miller, Victory Sports, GSS, and its agents, employees, counselors, volunteers, coaches, and any facilities they use, from liability on account of injuries sustained while participating in these activities. I give permission to be medically treated for illness occurring or injury sustained during such participation. I certify that I am covered by medical insurance which will reimburse Jason Miller, Victory Sports, and/or GSS for expenses incurred by them, their agents, employees, coaches, counselors, and volunteers on account of medical treatment ordered at their discretion, and also to indemnify them for any expenses not reimbursed by such insurance. In acknowledging consent on this form I hereby agree to abide by the expectations of Victory Sports & GSS, to be held accountable for these expectations by Victory Sports & GSS leadership, and grant Victory Sports & GSS the right to determine, and implement, any consequence to non-compliance. I also give permission for any media (pictures and/or videos) that involve the students I am registering, to be used for promotional purposes.
