CONTRACT INFO
This document provides information regarding the Portland Fighting Shockwave, (a 501(c)(3) company)(hereinafter "Portland Fighting Shockwave" or "Team"), a Member Team of the Women's Football Alliance (hereinafter "WFA"). A Portland Fighting Shockwave Player (hereinafter "Player") is a user of the Team and WFA's services, activities and facilities. Participation in Team programs is subject to the observance of Team rules and procedures. Any Player who violates these policies is subject to discipline as deemed appropriate by the Team, up to and including termination of her contract, and if applicable, prosecution under the fullest extent of the law. ATTITUDE
A positive attitude is essential to the Team's success and excellence as a football program. It is essential that the Player act in a professional manner and extend the highest courtesy to teammates, coaches, staff, volunteers, fans, opponents, vendors, and the WFA. To this end, a Player making negative or disparaging comments to or about any of the previously mentioned parties in any forum, including but not limited to social media (Facebook, Twitter, etc.) will be subject to discipline. PARTICIPATION
The Player will report on time for and participate fully in Fundraisers, Training Camps, Team Meetings, Practices and Community Outreach Events along with other team related events. The success of the Team is dependent on each Player's full participation and contribution to each event. The participation requirements are as follows: Pre-season: Wednesday, February 3rd - Friday, April 30th Reg. Season: Saturday, May 1st - Saturday June 19th Post-Season Sunday June 20th – Sunday, July 25th Each player is expected to fully participation from the start of Pre-Season through the end of the Post-Season for however long the Team is still participating. SCOPE
The Player is not an employee of the Team and does not have an employer-employee relationship with the Team. A Player's association with the Team is terminable at-will. A Player is free to discontinue her association with the Team at any time, for any reason or no reason, with or without notice. Nothing in this document is intended, nor should be construed, to alter the at-will nature of the Player's association with the Team or guarantee an association for any period of time. EXCLUSIVITY

EQUIPMENT
The Player is responsible for wearing appropriate safety gear during full-contact practices and games, including but not limited to: - helmet - chin strap - mouthpiece - shoulder pads - hip pads - tailbone pad - thigh pads - knee pads - cleats The Team can provide some of the equipment required to play the game of football including a helmet and shoulder pads. The Team can offer this equipment to the Player to borrow upon receipt of a $100 deposit (partial refund when gear is returned). The borrowed equipment must be returned, by the players doing, any time after the last official game and the End of Season BBQ. All other equipment is to be secured at the Player's discretion and is subject to review and approval by members of the Coaching Staff, the Team and the Game Officials. DRESS CODE
The Player is required to wear complete team uniform (as defined below) while participating in exhibition, pre-season, regular season or post-season games. They are also expected to wear Team gear (t-shirt, sweatshirt, etc.) for each pre-game and when participating in Team fundraisers and promotional events. Team uniform is defined by the following: Team Issued-Pre/Post-Game Day Gear: t-shirt, sweatshirt, shorts Team Issued-Game Uniform: jersey, pant, socks, belt and helmet sticker Team Authorized-Game Uniform: black base-layer *See Uniform Stick Girl for any other authorized uniform pieces Uniform Stick Girl document is available upon request Failure to adhere to the Uniform Policy will result in immediate suspension of participation until Uniform Policy is met. TEAM PROPERTY
At the end of the playing season, the Player must return to the Team all of the uniforms and equipment in her possession or issued to her by the Team three (3) weeks after the final game of the season or by the End of Season BBQ (whichever is latest). The Team may deduct from any payment due to the player, or bill the Player for the value of equipment not returned by the Player. The exception being "Pre/Post-Game Day Gear" (t-shirt, sweatshirt, shorts) and uniform socks. Please type your initials below.

TEAM FEES & WFA REGISTRATION
Upon signing of this contract the Player agrees to pay the following Fees to the Team: $25 Registration Fee + $500 Player's Fee (includes WFA Fee and Game Day Gear) There will be a $50 discount offered if fees are paid in full ($450) by the date of the first payment due (Friday Feb 28th)The fees are due according to the following payment fee plan: $25 due by Friday, February 5th, 2021$100 due by Sunday, February 28th, 2021 (1st of 4)$100 due by Wednesday, March 31st, 2021 (2nd of 4)$150 due by April 30th, 2021 (3rd of 4)$375 total due to be eligible for the 1st half of the regular season (beginning Sat May 1st, 2021) $150 due by May 30th, 2021 (4th of 4)$525 total due to be eligible for the 2nd half of the regular season and through the playoffs.These fees are non-refundable. If the Player has not paid her fees by these dates, the Player will not be able to continue to participate until payments are up to date and will not be placed on the roster for the 2021 season. Each Player is also responsible for registering with the WFA by April 15th 2021. The Team will cover each Player's WFA Registration Fee. Payment Plans are available upon request. Full compliance with the details of each payment plan must be followed. Missed payments will result in the Player not being able to continue to participate and / or removed from the roster for the 2021 season. INSURANCE

FITNESS
The Player represents to the Team that she is physically able to participate in full-contact football. If requested to do so, Player agrees that she will undergo a complete entrance physical examination by the Team. Player agrees to make full and complete disclosure of any physical or mental condition known to her which might impair her performance. INJURY

CONDUCT
The Player will abide by the Team's Code of Conduct. The Portland Fighting Shockwave, Portland’s PREMIER Women’s Full-Contact Football Team, is a community-based athletic and entertainment endeavor created to promote the sport of women’s tackle football in a positive, FAMILY environment and light. Participation in this organization’s programs is subject to the observance of the organization’s rules and procedures. Illegal activities of any kind while contracted for the Portland Fighting Shockwave are strictly prohibited. The Portland Fighting Shockwave recognizes its contracted participants are adults, and cannot therefore dictate to the extent it would like the actions taken by said participants. HOWEVER, any participant or staff member who violates this Code is subject to discipline as deemed appropriate by The Portland Fighting Shockwave, up to and including termination of his/her contract, and including when and if applicable prosecution under the fullest extent of the law: 1) Participants may not intentionally harass verbally or physically, or use abusive language towards, any staff member, volunteer or other participant. 2) Participants may not engage in behavior considered unacceptable in a family environment (i.e. excessive drinking, smoking, and/or activities considered grossly sexual in nature) while representing the team in any part of his/her uniform or in correlation with a Portland Fighting Shockwave Team Event, Team affiliate(s) event, or the Team or affiliate(s) logos/trademarks. 3) Participants (especially coaches and players) may NOT IN ANY WAY SHAPE OR FORM sexually harass anyone they come in contact with while participating in any Portland Fighting Shockwave event, either purposely or inadvertently. This includes lascivious comments and inappropriate touching, and when reported will result in IMMEDIATE action being taken. 4) Other abuses or misuse of authority granted a participant by the Portland Fighting Shockwave that are reported to/discovered by the administration will be handled on a case-by-case basis, but fall within this Code of Conduct as punishable. 5) Adherence to the WFA Code of Conduct is also required. PERSONAL RELATIONSHIPS

RIGHTS OF PUBLICITY
The Player grants the Team and the WFA, separately and together, the full authority to use her name, signature, likeness and/or pictures for all publicity and promotional purposes. The Player agrees not to engage in any promotional, advertising, sponsorship, or other activity for any entity or purpose without the prior written permission of the Team and the WFA. The Player may not use any logo, trademark and/or copyright of the Team and/or the WFA for any purpose without the prior written permission of the Team and the WFA.Please type your initials below.

PRIVACY
The sharing of information given internally to the members of the Portland Fighting Shockwave is strictly prohibited unless otherwise specified. This includes sharing of playbook, operational and other information not deemed relevant for use by the general public.
