NEW AND TRANSFERRING F-1 STUDENT AGREEMENT
I affirm that I have read the Reformed University F-1 Student Agreement and understand applicable policies and expectations.
F-1 STUDENT TRANSFER PROCESS
1. An F-1 student who is out of status and applies for reinstatement or transfers from another school must go through the typical admission process, pay all applicable fees and meet all the required criteria for admission to the Reformed University.
2. Acceptance into an Undergraduate or Graduate degree program is contingent on a student's ability to demonstrate proficiency in the English language. Reformed University does not guarantee acceptance into a degree program. Changing a degree program while studying at Reformed University requires a new application and repeating the admission process.
3. All students accepted into a Degree Program at the Reformed University receive an official acceptance letter. If you have received such a letter and wish to begin your study at Reformed University your exiting F-1 status must be transferred to your new school.
4. Please discuss the SEVIS transfer process and its effects on your legal status in the U.S. with your current school administration. This transfer will end Optional Practical Training (OPT) or Curricular Practical Training (CPT) automatically. Students who have an existing F-1 Visa and wish to transfer to another school in the U.S. must have their SEVIS record officially transferred from their current institution to the new institution (Reformed University) in order to receive an updated I-20 from the United States government. This is done by having your current school DSO (Designated School Official) complete F-1 Student SEVIS Record Transfer Form.

MASTER DEGREE REQUIRMENTS:
TUITION AND FEE INFORMATION
After signing the enrolment agreement and making payment students can request a refund. A dated and signed Tuition Refund Request Form must be submitted to the Registrar's Office. The Office of Business Affairs considers refund requests and considers extenuating circumstances such as student injury, prolonged illness or death, or other circumstances which prohibit completion of the course or program of study. To calculate a refund Reformed University uses the date on which the student initiates the official withdrawal or drop process as a benchmark. The date is made official when Tuition Refund Request Form is received by the Registrar's Office. Refund amounts are based on the proration of tuition and the percentage of the program completed at the time of withdrawal. Once the Tuition Refund Request is received and recorded by the Registrar's Office the refund schedule is as follows:
Most fees and charges for books and/or supplies are refundable with exception of the Application Fee and any items that were special ordered for a particular student or any third-party vendor costs. Refunds will be available within 30 calendar days of the refund request.
STUDENT AFFAIRS / INTERNSHIPS / PRACTICUMS
Though Reformed University does not guarantee job placement to its graduates the office of Student Affairs offers career counseling and informs students of employment opportunities in the community. Some degree programs contain internship and fieldwork requirements but all Reformed University students are eligible to receive help with career resources. Reformed University does not guarantee employment to graduates.
ACADEMIC CREDIT AND ATTENDANCE POLICY
Academic credit for a course requires both academic work and regular class attendance. Class attendance means being present in the classroom for the entire scheduled class session, whether or not any class assignments are completed or not. Students tardy by more than 15 minutes are marked as absent for the hour. Three tardy records count as one absence. Online classes at Reformed University follow the same attendance policy as campus classes and require weekly substantive interaction and assignment submissions which counts as class attendance. All students attending on-campus and/or online classes are required to attend at least 80% of the class meetings for each course in which they are enrolled. This means that a maximum of three class sessions may be missed during the typical 15-week semester. Absences due to late enrolment or add/drop change are excused but still count as absences. Absences due to illness and medical emergencies will be excused but still count as absences. Students who miss more than 20% of the class sessions for any reason will not be able to pass the course, will receive an “F,” for non-attendance, and will be withdrawn from the course. Successfully completed assignments have no bearing on grades for students who fail due to non-attendance. Exceptions to these policies may be made only in documentable cases of extreme and extraordinary circumstances and only with the explicit approval of the Reformed University administration.
I hereby affirm that I am entering into this agreement with the understanding of the terms contained herein.
Step 10. STUDENT DISCLOSURE AGREEMENT
I acknowledge that have not been misled in my inquiry for enrollment with the Reformed University and therefore I hold the University harmless from any of my own misunderstandings.
Step 11. STATEMENT OF FAITH AGREEMENT
Reformed University is an institution built on the foundations of historical Christian belief, ethics, and practice. All students, instructors, and staff members are required to understand and acknowledge this document.

Thank you for completing this application! Once your application and appropriate fee are submitted the admissions staff will be in touch with you regarding the decision. Please be advised that if admitted, depending on your individual application, additional documentation (such as official transcripts or other records) may be required to complete the enrolment process.
