Before you begin, you will need to ensure that you have the proper documentation.

To process a name change, TSLIC will require a marriage license, a divorce decree, or a court order.

We will also require a copy of your updated identification with your new name. We can accept a valid drivers license, state identification card, or a U.S. passport.

What Happens Next?

The Policy Owner will fill and sign this form and have one individual sign as witness to the Policy Owner’s signature. Return the completed information to Texas Service Life Insurance Company. When executed by the Company, a copy will be sent to you for your records.
