 Acknowlegement of Terms and Release of Liability

I will receive a recurring invoice and submit payment for dues each month unless I enroll in automatic monthly payments. 

Special events and performances may be offered periodically.  I agree to allow my child to participate in special performances and to provide my child with uniforms, supplies, and transportation as needed.

I give my permission to Maui Music Mission to record images of my child while performing activities with the After School Music Club, and to post those images on social media and other platforms for the promotion of Maui Music Mission. I further agree to hold Maui Music Mission harmless of any and all liability or damages associated with my child's enrollment in the program.

I acknowledge and accept the terms and conditions for enrollment in the Maui Music Mission After School Music Club
