You are completing this form to be a participant of a Take A Soldier/Veterans/Active Duty Fishing event. As a participant, you are certifying that you are serving or have served as a member of the United States armed services (any branch, Air Force, Army, Coast Guard, Marines, National Guard, Navy) and that you have a valid Military ID card or a DD214. Some events have a limited number of participants and may have a waiting list. For the Hagg Lake event, please notify event coordinator Leroy Bentley, bentley16@frontier.com or (503-502-1051) if you cannot attend the event after being confirmed as a participant. Please make the cancellation notification as early as possible so someone else can take your place. Please notify the event coordinator if you will arrive late or need to leave before the advertised event times. After completing the form, the event coordinator should notify you within 48 hours regarding your event registration status and provide additional event information.
