This form should be completed by the designated Trip Leader for club-approved off-campus trips including recreational activities, preseason sporting tournaments and instructional camps (but not for competitive fixtured games or training).
Completing this form will assist with:
- The University supporting the Club to deal with a critical incident
- Potential Personal Accident Insurance claims
- MU Sport promotion of the trip.
Club Trip Leaders are encouraged to submit this form a minimum of 48 hours before the trip departs. Club Trip Leaders must also be aware of MU Sport's Club Critical Incident Procedure.
After submitting, Club Trip Leaders will receive an automated email confirming that their Club Trip Notification has been received.
