Karate, tuite and martial arts instruction is a physical training regimen that can result in injury to the practitioner. All care during instruction will be taken to avoid injury and all efforts will be made toward safe training. The student (or parent, if the student is under 18) assumes full responsibility for any and all injuries or damage which may occur to the student and does hereby fully and forever release and discharge Tyler Karate Academy, the All Okinawa Shorinryu Karate and Kobudo Federation, the Okinawa Shorin Ryu Matsumura Orthodox Karate and Kobudo Federation, Fusei Kise, Isao Kise, Nathan Batson and any other owners, representatives, officers, servants, agents, or members from any and all claims, demands, damages, rights of action, or causes of action, present and/or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of the students use or intended use of said facilities, members, and equipment thereof, without limitation, any claims for personal injuries resulting from or arising out of the negligence of school, its officers, representatives, servants, agents, or members.
Visitors are welcome and encouraged to watch the training at no charge. Videotaping will not be allowed. Please respect this request. In the event photos or video are taken (with permission by the organizers), by signing below, you give your consent to make the photos or videos available to the camp organizers and their designees for training and/or advertising purposes.
I have read and understood the preceding two paragraphs and agree to receive instruction under these conditions and at my own risk; if I am the parent of a camp attendee who is under 18, I agree to the same on behalf of my child. I agree to conduct myself with decorum and protocol with high regard for the safety of my fellow camp participants during this event and understand that failure to do so will result in my expulsion from the event, without refund.
