2020 DecoTurf High School Tennis Team Championships Deposit

Please note that making this deposit will only guarantee your team a spot in the 2020 DecoTurf High School Tennis Team Championships IF you meet the following criteria...

• Have submitted the 2020 Participant Interest Form at https://form.jotform.com/hstennischampionships/2020-decoturf-interest-form.
• Have a UTR Power 6 rating, the combined UTR of your 6 highest rated players, of 45 or higher for Boys or 30 or higher for Girls.
• Are a member of your state high school athletic association. In most cases this means that independent schools are not permitted to play in our event. If you have questions about this email the Tournament Director at hstennischampionships@gmail.com.

Please note that if you fail to meet any of the criteria above that you may NOT be selected for the event even if we receive your deposit.

All tournament deposits are NON-REFUNDABLE!

PAYMENT

If you are bringing both boys and girls please be sure to make fill out this form and make two seperate deposits.

You can also submit your payment via check to HS Tennis Championships, 13611 Tennis Blvd, Louisville, KY 40272. If submitting your payment by check please print this form and include with your payment, please do NOT submit the form if paying by check. If you would like to pay by credit card you can submit your payment below.
