Welcome to the entry form of Spooks & Sparkles Halloween Pageant!

We are excited to have you apart of the fun day. Our Spooks and Sparkles Halloween Pageant is available for Natural and Glitz. If you have any questions, feel free to email Amy at midwestfabulousdolls@live.com

Packages: The Supreme packages are great purchases. They save money and also you will be able to enter each thing. If you do not want to purchase the Supreme package, you must purchase the Registration fee and mandatory T-shirt purchase. Both are down below to purchase. Our non-talent packages are available for those who do not want to do talent. 

Deadline to enter is October 5th. No exceptions! Can't wait to see what you're going to come up with to win the title of your division and your chance to win the Supreme title! :)

Category Definitions/Rules:

Glitz Supreme Package Ages 0-18 years: Will cover Registration, Outfit of Choice Halloween Wear, Beauty, Introduction, Wow Wear, Talent, Comp Card, and Side Awards: Best Smile, Beautiful Eyes, Most Beautiful, Prettiest Hair, Most Photogenic, Best Dressed, Best Costume, Most Talented and Special Awards: Director's Choice, Viewer's Choice.

Natural Supreme Package 0-18 years: Will cover Registration, Outfit of Choice Halloween Wear, Beauty, Introduction, Wow Wear, Talent, Comp Card, and Side Awards: Best Smile, Beautiful Eyes, Prettiest Hair, Most Beautiful, Most Photogenic, Best Dressed, Best Costume, Most Talented and Special Awards: Director's Choice, Viewer's Choice.

Non-Talent Package 0-18 years: Will cover Registration, Outfit of Choice Halloween Wear, Beauty, Introduction, Wow Wear, Comp Card, and Side Awards: Best Smile, Most Beautiful, Beautiful Eyes, Prettiest Hair, Most Photogenic, Best Dressed, Best Costume, and Special Awards: Director's Choice, Viewer's Choice and free T-shirt.

Ages 0-3: Talent is available for 1 minute.

4-18 years: Talent is available up to 3 minutes. This is an optional. You can join the talent optional if you purchase the Supreme Package. If you do not want to join the talent optional, you may purchase the Non-Talent package. 

You are allowed to do both Glitz and Natural. In order to do so the best thing to do is to purchase both packages to have better chances of winning. If you want to do just one, that is okay. 

If you do not want to purchase a package, you must purchase Registration Fee. That contains Beauty, Introduction, Interview, and Comp Card. To enter other categories, you must purchase that option to enter.

Beauty: You will need to wear a formal dress. May be an Easter dress, prom styled dress, ball gown, etc. If you are doing Natural, you'll need a natural style dress. If you are doing Glitz, you'll need a Glitzed styled formal dress. Hair and face needs to impress as well! :) Hair will be judged towards Prettiest Hair for those who purchased Supreme packages or purchased the Prettiest Hair Category.

Outfit of Choice Halloween Wear: This category is what you would wear the night out to go Trick or Treating. If you are doing both packages, you will need to find 2 outfits that fit the Natural look and Glitz look. Remember, better chances at winning! :) Also please try not to be too scary looking, we do have children who may be afraid of a certain character, etc.

Wow Wear: This category would be something you would wear that is Fall/Halloween themed. Can be a casual outfit, a cute dress, etc. Same thing goes towards Natural and Glitz apparels. Know the differences!

Director's Choice: Impress the Director with your overall pageant appearances! She will go by each age division.

Viewer's Choice: Impress the crowd with your overall pageant appearance. They will go by age division as well.

Most Beautiful: This will come from Beauty!

Best Smile: Do you have the smile that may melt the judges heart? :) 

Best Eyes: Impress the judges by showing of the set of your eyes when you do all categories.

Prettiest Hair: This will be going by your Beauty category. Impress us!

Most Photogenic: This will be coming from your Comp Card. You can easily fit 4-6 pictures on a 8x10 hard photo paper. The judges will judge you by your poses, outfit choices, facial expressions, beauty, and how the Comp Card is layed out. Good luck! :) 

Best Costume: Do you think you have the best costume out of your age division? Remember, this is eligible for Natural and Glitz! If you are doing both packages, you'll need an outfit for Natural and one for Glitz. Please do not go overboard with your outfits, some children may be afraid of some characters. Props are welcome!

Best Dressed: This will go by your Wow Wear. Impress us!

Most Talented: Available for all ages. 0-3 years can go up to 1 minute. 4-18 can go up to 3 minutes and show us how you express your talent! Please bring your CD along if you are performing with music. If you are singing, it must be non-vocal! Be creative!

Door Admission: Contestant and 1 adult helping are free. If you'd like to bring a family member, friend, etc. they will need to purchase door admission according to their age category. Door Admission Fees will be going towards location fees. Would appreciate the help there, thanks!

Pageant T-shirt: Contestant automatically gets a free t-shirt in the Supreme Package. If you purchase both Supreme packages, you only get one t-shirt. If you don't purchase a Supreme package, you are mandatory to purchase a t-shirt for this pageant. They are $15, check the box if you need one. T-shirts are also available for those who are not a contestant. 

Awards: These are the approximate amount of awards per category/divisions/optionals. We have every right to lessen the amount if there is a low number of contestants. Awards are for Natural and Glitz seperately. Can be double crowned if purchased both packages. Crowns, sashs, certificate, and trophy will go to the top Supreme winners! No one leaves empty handed! 

Divisions: 0-1, 2-3, 4-6, 7-9, 10-12, 13-15, 16-18

1 Supreme Title: The Grand Supreme is the ultimate title in any beauty pageant. It is awarded to the one contestant with the highest combined score in the pageant. This is the shining star that contestants shoot for. This is best when purchasing a package! 

1 Mini Grand Supreme: This title is awarded to the next girl in line for the Grand Supreme title. She is the first runner up for the biggest title in the pageant.

7 Divisional Supremes: This is the contestant with the highest combined score in her age group in the pageant.

Optionals: A contestant from each age division will be called out for this specific award! A sash, trophy, and a certificate will be handed out with these.

We accept PayPal for credit/debit card transactions. We also accept cash and money order methods as well. Please read below for directions on how to pay for fees.

If you are interested in helping out with the pageant as an emcee, door admission, judging (if you have a child that is a contestant, you can not judge), etc., please email Amy at midwestfabulousdolls@live.com for more information.

We are looking for volunteers to help out with munchies, treats, drinks, etc. for this event. If you would like to donate to make this Halloween Pageant amazing, that would be great. Candy would be appreciated so we can fill the goody bags up for that as well! Let Amy know! :)

Payment methods:

We accept PayPal for credit/debit card payment method. If you do not have an account, they will still accept without creating one. To avoid processing fees, please send payment as gift. Our Paypal email is midwestfabulousdolls@live.com! If you do not want to pay via PayPal, we accept cash or money orders. Please write the money order out to Amy Meyer and send to 3220 170th St. Manson, Iowa 50563. Make sure your name is on the money order or it can not be processed.  Once you pay, there is no refund. We start ordering products for each contestant. If there is any reason you can not attend, your payment can be credited to any future events we provide. We have every right to postpone the event for any reason and you will be notified the same day. Thanks!

Midwest Fabulous Dolls Pageant Release Form:

As the parents (legal guardian) of the participant named above in an official Midwest Fabulous Dolls Pageant, I do hereby agree that if my child wins the State/Preliminary pageant she is obligated to represent that title at the national pageant. Should we elect not to represent that title at the national pageant, I understand that we forfeit all prizes and rights awarded to that title. I do further agree to abide by the rules and regulations, directives and provisions for the winners and participants as set forth in the official Midwest Fabulous Dolls Participant Handbook and by the director of Midwest Fabulous Dolls. I agree to hold America’s Fabulous Faces, Midwest Fabulous Dolls Pageants, and its directors and staff members harmless of all damages due to theft, accident or injury during or resulting from my child‟s participation in the State pageant. I accept the responsibility for the regulation of hours, personal conduct and activities of my child and affirm that the enclosed forms are true to the best of my knowledge and belief, including birth date, city and state of residence. I further understand that any false or misleading information will be sufficient reason for dismissal from the pageant without a refund. I accept that when I pay for fees of the pageant, there is no refund. I accept that the non-refunded amount will be waived over to the next upcoming pageant available.

By signing this you agree to good sportsmanship and the use of all pageant photos to be used by the pageant system. You agree to the terms that the pageant director, pageant system, and pageant venue is not in anyway responsible for losses, theft, or damages to any personal property or to any personal injuries.

Photograph/Media Copyright Release Form

All of photographers of each pageant that is provided by Midwest Fabulous Dolls gives permission for the reprint of the photographs/media of you, the contestant, to be used for the Midwest Fabulous Dolls or America’s Fabulous Faces for website, publicity, and/or program book, etc. Midwest Fabulous Dolls may change the rules to where cameras can not be allowed if photographer/videographer does not allow. All professional photographs/media submitted for the Midwest Fabulous Dolls must be accompanied by a release giving permission to reprint the photographs. You may use this release, or any release provided by the photographer.
