• Community Service Contract

  • This Community Service Agreement (this "Agreement") executed on the {day} day of {month} {year} by and between the parties:

    Great Community Society, with principal address at 3128 Doctors Drive, Los Angeles, California, 90017 (hereinafter known as "Organization"); and {nameOf}, with address at {addressOf:addr_line1} {addressOf:city} {addressOf:state} {addressOf:postal} (hereinafter known as "Volunteer").

  • WHEREAS, Volunteer is willing to offer his/her time and services that benefit the community;

    WHEREAS, Organization provides services to the community for free and likewise accepts volunteers to be part of its community workers;

    WHEREAS, the Volunteer desires to provide services to the community through the capacity of the Organization, and Organization accepts the services and time offered by Volunteer.

    WHEREFORE, in consideration of the pronouncements above, the parties hereby agree to the following terms and conditions:

  • Purpose of Participation

    The Volunteer participates in the activities of the Organization for {volunteerismParticipation} community service purposes. 

  • Conditions for Community Service

    For the purposes of volunteerism by court order, it is understood by the Volunteer that a volunteer must not have been charged, accused, or convicted of criminal offenses involving the following:

    1. Committing acts of violence or threats of violence
    2. Illegal possession of dangerous weapons
    3. Possession of illegal substances
    4. Inappropriate sexual behavior
    5. Theft or fraud in the last 10 years.

    In the event that such volunteer act is due to court order, Volunteer shall disclose the purpose and the offense or infraction committed for which community service is required. It shall be understood and shall be the responsibility of both parties that any discussion or information exchanged between them shall be kept in confidence. 

    It shall be the responsibility of the volunteer to furnish the Organization the official documents pertaining to the order for community service, the decision of the court in relation to the offense charged, any plea bargain agreement or motion to dismiss the charges and appeal. During the period of service, Volunteer shall keep the Organization updated relating to any decision made by the court.

     

  • Volunteering Work Period

    Any activity made by Volunteer must be with approval and/or supervision of a staff by the Organization. Any activity outside the presence of the Organization's staff may not be certified.

    Volunteer activities shall be conducted within a reasonable time and period. Accomplishments and daily activities shall be recorded as part of the Organization's reportorial duties. Organization reserves the right to increase or decrease hours for a period provided that Volunteer is given prior due notice.

  • Relationship

    This Contract shall not, any instance, be considered as constituting an employer-employee relationship. The existing contractual relationship between the the parties herein shall not be affected by the effectivity of this agreement. 

  • Privacy

    The parties understand that any information shared by them to each other shall be kept private. The Organization may share or disclose information and records received and made by them for reportorial purposes only to stakeholders. 

  • Separability

    In case any of the provisions found in this agreement is held invalid or unenforceable by a competent court, such invalidity or unenforceability shall only affect the said provision and the rest shall remain valid and enforceable. 

  • Precedence

    This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof. Any agreement previously made in relation to this matter whether made orally or written shall be superseded by this Agreement.

  • Signed by the parties on the date specified.
