Please read through the clauses below and sign the form in the field provided as an acknowledgment of your understanding of the terms and conditions of this Contract.
A. Amount of Registration Fee.
The anticipated registration fee for the {number} season is $100.00. The registration fee covers the following:
The registration fee covers the following:
- League and Cost of Referee fees;
- Administrative fees; Association Fees;
- Operational and overhead costs which include medical support and the cost of rentals.
B. Mode of Payment:
The fee shall be paid as follows:
Installment:
20% upon registration;
80% final payment anytime before the end of season;
Full Payment:
100% upon registration
C. Withdrawal:
Player may withdraw after registration anytime before the beginning of the season for any reason without penalties against the player. Upon the start of the season, a player may withdraw for valid grounds and which shall be deliberated by the committee. However, no fee shall be refunded back to Player.
D. Training:
In the event that Player missed a training session, Player shall not be allowed to start the next game thereafter, unless absence is excused.
E. Injuries:
in case injury occurs and Player is unable to play for a period of more than 8 weeks or more, Player shall be withdrawn from the season. Payments made shall be refunded with 20% deduction from the total amount of fees for the season.
F. Conduct:
Players shall maintain good conduct and sportsmanship in respecting the rules, the officials, and their decisions. Any actions by Player to the contrary shall be met with penalties including but not limited to damages or expulsion.
Player understands that the Player's membership is being accepted with the understanding that the Player and their representatives will be responsible for the cost of registration, including the compliance with the terms and conditions as provided herein this Agreement.
