Programme Delivery Contract

Please answer the following questions as accurately as possible. This will help the team at Mind of the Student and our delivery partner, Working Options in Education, process your booking request more efficiently. We look forward to visiting your school soon.

This contract must be co-signed by a member of your Senior Leadership Team (SLT). If you are already a member of SLT, please put N/A.

It's imperative that our workshops are delivered on time to avoid important content being missed. We now require a point of contact at the school, to lead primarily on crowd control and student ushering, to keep the day running smoothly and on time. This MUST be a different person who has not yet been added to this form. The Timekeeper will ensure that students arrive punctually and are seated in the hall/venue by the designated time, as outlined in the provided timetable.

Please confirm this date with your team before proceeding, as we may be unable to change your workshop date once this form is submitted.
