Studio 91 Waiver
Parent/Guardian Information
At Studio 91, all participants ages 2 to 5 years of age must have a parent/gaurdian present with them at all times. Participants who are ages 6 to 15 may participate in our drop-off workshops and after-school programs without a parent/gaurdian. Please list all who are authorized to pick-up your child. Please also give us a copy of his/her photo ID to have on file, we will ask for it prior to releasing any child for pick-up.

Photo Waiver & Release
I hereby give my permission to Studio 91 to use photos taken of my child at the studio as well as photos taken of their created artwork to be used for local news and marketing purposes, including on Studio 91's social media channels.
Informed Consent and Acknowledgement
I hereby give my approval for my child’s participation in any and all activities prepared by {Studio 91} during the selected workshop, class or camp. In exchange for the acceptance of said child’s candidacy by {Studio 91} ., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless {Studio 91} . and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.
In case of injury to said child, I hereby waive all claims against {Studio 91} . including all teachers and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.
