1. I hereby assume all risks of participating in the above mentioned activity.
2. I hereby agree to waive, release and discharge PSB Academy, their staff, organisers, officials, agents or volunteers from all liabilities, including but not limited to, liabilities for death, personal injury (whether fatal or otherwise), loss of damage to property and any other losses, damage, cost and expense whatsoever and howsoever arising out of or in connection with my participation in the abovementioned activity.
3. I shall abide by all the rules, regulations, instructions and requirements related to the above mentioned activity.
4. I confirm that I have no health related problems that will prevent me from participating in the above mentioned activity.
5. I confirm that the information given below is true and changes made after registration will not be entertained.
