Thank you for applying to be a model for Sunshine Photography. This questionnaire will determine if you are a good fit for the model call at this time. If not, we will file your questionnaire for a later date and contact you if we need you at that time. Please fill out the form honestly and in full. You will receive a response via email if you are chosen. Please make sure you check your spam folder as sometimes emails go there if not from a frequent contact. By filling out this form you are acknowledging that you must pay a $15 refundable retainer that will be returned back to you after your session or you can use it towards more images. You also will receive 3 High resolution Images for your time and 50% off any other image products if you wish. There is no obligation to purchase anything additional.
