  • I hereby certify that my son/daughter listed above has my consent to attend any and all activities and programs sponsored in coordination with the Office of Youth Ministry at Sts. Peter and Paul Church, New Braunfels Texas for the calendar year 2019-2020. I hereby release the Office of Youth Ministry, Sts. Peter and Paul Church, and the Archdiocese of San Antonio from any liability for injuries or fatalities suffered by my child while he/she is under the supervision of the sponsors of these activities.

  • I hereby give my permission for my students listed above to appear in youth ministry advertising such as, but not limited too, photos and videos.
