AMP Camp is an arts, music, and performance camp for children entering grades 3 through 8.
Dates: July 24-27, 2018
Times: 9 a.m. to 3:30 p.m.
Cost: Early registration. $120 per child After July 1st. $150 per child
I/We being the parents or legal guardians of the student mentioned above, do further give my/our consent for the class leaders of The Mission to secure the administration of the medical treatment or medication for the above named child, and I/we do further agree to the performance of such treatment, anesthetics, and operations as in the opinion of the attending physician that is deemed necessary for our child. (At the bottom of the page list any medication or treatment that should NOT be given to your child because of dangerous reactions.)
I, Parent/Legal Guardian of student mentioned above hereby grant permission to The Mission of Vacaville, CA and its assigns and licensees to take photographs or videos of the above and to make recordings of the above named minor child's or dependent adult's voice as indicated below:
By GRANTING permission as directed above, I am giving The Mission permission to use these images, videos, and recordings, of my likeness and voice as follows:
I further acknowledge that I will not be compensated for these uses, and that The Mission exclusively owns all rights to the images, videos, and recordings, and to any derivative works created from them.
I waive the right to inspect or approve the uses of any printed or electronic copy. I hereby release The Mission and its assigns and licensees from any claims that may arise from these uses, including without limitation claims of defamation or invasion of privacy, or of infringement of moral rights or rights of publicity or copyright.
This release is binding on me, my heirs, assigns, and estate. The Mission is not obligated to use any of the rights granted under this Release. This release expresses the complete understanding of the parties.
Registration costs $120 if paid in full before July 1, 2018. After July 1st, registration goes up to $150 per student. If you are a Mission Family Pass holder then be sure to check your email for a special discount promo code.
If you pay the $40 deposit costs then the rest of your registration fee will be due before July 1. A link will be provided in your confirmation email that can be used to pay your remaining balance. This link can also be found on our website.
