PURPOSE: Here at UCC, we seek to provide everyone with an opportunity to encounter Jesus Christ and experience the great love and grace that God has shown us all. One way that students encounter Jesus is through special events, conferences, and trips that we take as a youth group. We want to allow students the opportunity to attend such events if they so desire. We understand that finances can be an obstacle at times. We never want this to prevent students from attending events with us. The church has been blessed and wants to bless others in return. God has given to us therefore we are able to give.
INSTRUCTIONS: Please have the student who wants to attend fill out this application completely, sign, and date it. It also needs to be reviewed, signed, and dated by a parent or guardian. (All items with a red asterisk are required.)
