Dance & Cheer Lesson Liability Waiver and Agreement

1. Acknowledgment of Risk and Release of Liability

I, the undersigned participant or parent/guardian of the minor participant, understand and acknowledge that dance lessons and physical activity inherently carry the risk of injury. By signing this waiver, I voluntarily assume all risks, including but not limited to, physical injury, and I release and hold harmless Jordan Taylor, all instructors of Youth Dance and Cheer, and Total Balance Massage and Wellness(collectively “the Released Parties”) from any and all liability, claims, or actions arising out of any injury, accident, or harm suffered in connection with participation in dance lessons, practice, or use of the facility.

2. Agreement Not to Sue

I agree that I, my representatives, or anyone on behalf of the participant will not pursue legal action against the Released Parties for any injury, loss, or damage incurred during or as a result of dance lessons or activities on the premises, except in cases of gross negligence or intentional misconduct.

3. Defamation Clause

I also agree that I will not make any false or defamatory statements, in person or through any media, that could harm the reputation of the instructors, the Released Parties, or their business operations. I understand that any breach of this agreement may result in legal action, including for defamation of character, by the Released Parties.

4. Behavior Policy and Right to Refuse Service

I understand that instructors reserve the right to refuse service or discontinue lessons for any participant whose behavior is disruptive, harmful to the facility, instructors, or other students, or poses a safety risk. If such behavior occurs, I acknowledge that lessons may be terminated without refund.

5. Tuition Payment and Refund Policy
I understand and agree that tuition is due in full on the first day of each month. If payment is not received by the due date, I acknowledge that my child’s spot in class may be forfeited to another student.

I acknowledge that all tuition payments are non-refundable, regardless of attendance, missed classes, or withdrawal during the month.

If I wish to cancel my child’s enrollment and stop future payments, I agree to provide written notice via email to cr8tvedance@gmail.com prior to the next billing cycle. Verbal cancellations or text messages will not be accepted as official notice.

By signing below, I agree to abide by this payment and refund policy as a condition of participation in Youth Dance and Cheer programming.
