Be Honest: The self-assessment you complete may be considered a discoverable document in legal contexts. Any dishonesty or manipulation of information will lead to inaccurate results.
Use a Third Party if Needed: If you believe you may not be able to provide an unbiased assessment, consider using a third party or an unbiased employee/volunteer to complete the form on your behalf.
Best Practices:
- Risk audits should be conducted after major changes, incidents, or accidents.
- At a minimum, audits should be performed annually around the same time.
- Each location should have its own audit to reflect its unique needs.
Purpose of the Audit: This self-assessment is designed to gauge your current risk levels accurately. The goal is to provide your facility with actionable insights to improve safety and security, protecting your people, brand, and bottom line.
Importance of Accuracy: Fudging numbers or being dishonest compromises the audit’s effectiveness. Accurate information ensures you receive the best recommendations for upgrading your safety and security measures.
To help us accurately assess the safety and security of your facility, please use the following scale to rate each statement based on your observations and experiences.
1-Strongly Disagree: The statement is not true at all; the condition is very poor or entirely unmet.
2-Disagree: The statement is mostly not true; the condition is frequently poor or unmet.
3-Sometimes: The statement is true occasionally; the condition is sometimes met but not consistently.
4-Agree: The statement is mostly true; the condition is generally good or mostly met.
5-Strongly Agree: The statement is completely true; the condition is excellent or always met.
N/A-Not Applicable: used when a piece of equipment or policy is not present in your facility, or if a question does not apply to your business.
NOTE: In the audit where it references Plans, policies, procedures, rules, or guidelines - this means they are written and your staff has working knowledge of them.
