    Purpose of the Audit: This self-assessment is designed to gauge your current risk levels accurately. The goal is to provide your facility with actionable insights to improve safety and security, protecting your people, brand, and bottom line.


    Importance of Accuracy: Fudging numbers or being dishonest compromises the audit’s effectiveness. Accurate information ensures you receive the best recommendations for upgrading your safety and security measures.

    To help us accurately assess the safety and security of your facility, please use the following scale to rate each statement based on your observations and experiences.

     

    1-Strongly Disagree: The statement is not true at all; the condition is very poor or entirely unmet.


    2-Disagree: The statement is mostly not true; the condition is frequently poor or unmet.


    3-Sometimes: The statement is true occasionally; the condition is sometimes met but not consistently.


    4-Agree: The statement is mostly true; the condition is generally good or mostly met.


    5-Strongly Agree: The statement is completely true; the condition is excellent or always met.

     

    N/A-Not Applicable: used when a piece of equipment or policy is not present in your facility, or if a question does not apply to your business.

  • The staff parking area is well-lit and close to the building entrance, has security measures in place such as cameras or security patrols, and has designated parking spaces for staff to ensure organization and safety.*
  • The visitor parking area is clearly marked and easy to find, has adequate lighting and security measures in place, and has signs directing visitors to the main entrance from the parking area.*
  • The yard/playground area is securely fenced to prevent unauthorized access, the play equipment and grounds are regularly inspected for safety, and there are supervision policies in place to ensure children are always monitored while in the yard/playground area.*
  • BUILDING

  • The front of the building/main entrance is well-maintained, free of obstructions that could block visibility, clearly marked with signs directing visitors to the main entrance, and monitored by security measures such as cameras or security personnel.*
  • The emergency staircase is well-lit to ensure safe use during an emergency, the stairs and handrails are in good condition and free from hazards, and regular fire drills and emergency evacuations are conducted to familiarize everyone with the use of the emergency staircase.*
  • All doors that lead to the outside (back and sides of the building), are secure and equipped with appropriate locks to prevent unauthorized access, easy to open from the inside to ensure quick evacuation if needed, and monitored by security measures such as cameras or access control systems.*
  • The exterior lighting is sufficient to illuminate all areas around the building during the night, the lights are positioned to avoid creating shadows or dark spots that could conceal potential threats, and the exterior lights are regularly checked, audited and maintained to ensure they are working properly.*
  • The exterior windows are in good condition with no cracks or damage that could compromise security, there are locks or other security measures in place to prevent unauthorized entry through the windows, and the windows are covered by security cameras to monitor any suspicious activity.*
  • The fuel tank (natural gas/ propane) is securely located and protected from potential tampering or vandalism, there are safety measures in place to prevent and respond to fuel leaks or spills, and the fuel tanks are regularly inspected and maintained to ensure they are safe and secure.*
  • The lock box/Knox box is securely installed and accessible only to authorized personnel, the keys or access codes stored in the lock box/Knox box are up to date and accurate, and regular checks are performed to ensure the lock box/Knox box is functioning properly and remains secure.*
  • INTERIOR

  • The admin office/staff office is secure and accessible only to authorized personnel, sensitive documents and equipment are stored securely when not in use, and the office is equipped with security measures such as locks or access control systems.*
  • The cash register area is monitored by security cameras to deter theft, cash handling procedures are in place to minimize the risk of loss or theft, and the cash register/cashier area is secure and protected from unauthorized access.*
  • Classrooms are equipped with locks to secure them when not in use as well as additional locks for lock-downs, emergency procedures such as evacuation plans are clearly posted and practiced regularly, and there is a communication system in place for teachers to quickly contact the front desk or administration in case of an emergency.*
  • All interior doors are in good condition and equipped with functioning locks to include additional locks for lock-downs, doors are labeled clearly to indicate their purpose and the rooms they lead to, and there are procedures in place to ensure doors are locked when rooms are not in use.*
  • Emergency exits are clearly marked and easily accessible, free from obstructions that could impede evacuation, and regularly checked to ensure they are functioning properly.*
  • The staircase is well-lit and free from hazards, the stairs and handrails are in good condition and safe to use, and exits and routes of egress are clearly marked.*
  • The indoor activity areas are well-maintained and free from hazards, the gym/sports area is regularly inspected to ensure equipment is safe and in good condition, there are procedures in place to quickly address any injuries or emergencies, and the area is monitored to prevent unauthorized access and ensure the safety of participants.*
  • The janitorial/maintenance/utility room is securely locked when not in use to prevent unauthorized access, hazardous materials are stored safely and according to safety regulations, and there are procedures in place to handle spills or emergencies in the janitorial/maintenance room.*
  • The locker room is monitored to prevent theft and ensure safety, lockers are in good condition and equipped with functioning locks, and there are clear rules and guidelines for locker room use to maintain order and safety.*
  • The lounge/parent area is well-maintained and free from hazards, there are security measures in place to prevent unauthorized access, and the lounge is monitored to ensure safety and prevent misuse.*
  • The reception area is well-lit and welcoming for visitors, there are security measures such as cameras or access control in place to monitor the reception area, and the reception area is designed to manage visitor flow efficiently and securely.*
  • The reception desk is staffed during operating hours to assist visitors and monitor access, there are security measures in place such as panic buttons or communication systems for the reception desk staff, and the reception desk staff is able to at least passively monitor critical CCTV shots of the facility such as the front door and other open access points.*
  • Restrooms are regularly cleaned and maintained to ensure hygiene and safety, and there are security measures in place to prevent unauthorized access or misuse of restrooms.*
  • The supply closet is securely locked when not in use to prevent unauthorized access, supplies are organized and stored safely to prevent accidents, and regular checks are conducted to ensure the supply closet is well-maintained and secure.*
  • The playscape area is regularly inspected to ensure the equipment is safe and in good condition, there are clear rules and guidelines for using the playscape to ensure safety, and the playscape area is monitored to prevent accidents and ensure children are supervised.*
  • EQUIPMENT

  • Monthly equipment safety checks, conducted according to manufacturer specifications, and quarterly facility safety inspections, guided by best practices, are routinely performed and thoroughly documented.*

  • The AED is easily accessible with a < 3 minute response time from anywhere in your facility to the pateint and clearly marked in case of an emergency, staff are trained in using the AED, and the AED is regularly checked and maintained to ensure it is in working condition.*
  • Duress alarms/panic button devices are easily accessible to staff in case of an emergency, staff are trained on how and when to use the duress alarm/panic button, and the devices are regularly tested to ensure they are functioning properly.*
  • Emergency lighting is installed in all key areas to ensure visibility during a power outage, regular tests are conducted to ensure emergency lighting is functioning properly, and backup power sources are available for emergency lighting.*
  • Facility keys are securely stored and accessible only to authorized personnel, there are records of who has access to facility keys or who has been issued facility keys, and procedures are in place for handling lost or stolen keys.*
  • The fire alarm system is regularly tested to ensure it is functioning properly, there are clear procedures for evacuating the building when the fire alarm sounds, and staff and children are trained in fire drill procedures.*
  • Fire extinguishers are easily accessible and clearly marked throughout the facility, staff are trained in using fire extinguishers, and fire extinguishers are regularly inspected and maintained.*
  • The first aid kit is fully stocked, no expired items inside and easily accessible in case of an emergency, staff are trained in basic first aid procedures, and the first aid kit is regularly checked and restocked as needed.*
  • The gun detection and recognition Ai system for exterior cameras is functioning properly to monitor person(s) with guns in the parking lot and approaching the facility, procedures are in place to respond immediately to threats, and the data from the Gun Detection and recognition system is shared with police.*
  • There is an incident/ Injury management system in place to report and track incidents and injuries, staff are trained in using the incident/ Injury management system, and incidents are reviewed and addressed promptly to prevent recurrence.*
  • The license plate recognition system is functioning properly to monitor vehicles entering and leaving the facility, procedures are in place to respond to unauthorized vehicles, and the data from the license plate recognition system is regularly reviewed.*
  • The PA system/mass notification system is tested regularly to ensure it is functioning properly, there are clear procedures for using the system to communicate during emergencies, and staff are trained in operating the PA system/mass notification system.*
  • Smoke detectors are installed in all key areas of the facility, regularly tested and maintained to ensure they are functioning properly, and there are clear procedures for evacuating the building when smoke detectors are activated.*
  • Two-way radios are available and functioning for staff to communicate during emergencies, staff are trained in using two-way radios, and there are regular checks to ensure the radios are charged and in working condition.*
  • The CCTV system covers all key areas of the facility, the cameras are regularly checked and maintained to ensure they are functioning properly, and the surveillance footage is regularly reviewed to identify any security concerns. Front desk staff are able to passively monitor all open entrances.*
  • The visitor management software or process is functioning properly to track visitors entering and leaving the facility, procedures are in place for managing visitor access and ensuring they are properly identified, and staff are trained in using the visitor management software or the process. Visitors are never left alone in the facility.*
  • The Wi-Fi/internet connection is secure and functioning properly, measures are in place to prevent unauthorized access to the network, and regular checks are conducted to ensure the network is secure and performing well.*
  • The air conditioning and heating systems in this facility are up-to-date and provide a safe environment for students in all weather conditions.*
  • PERSONNEL

  • Security guards are trained and certified according to industry standards, conduct regular patrols of the facility to ensure safety, and follow clear procedures in case of an emergency or security incident.*
  • Thorough background checks are conducted on all employees and volunteers before they are hired, background checks are updated periodically to ensure ongoing security, and there are clear policies for handling any red flags that arise during background checks.*
  • There is a designated security coordinator responsible for overseeing all security measures, the security coordinator regularly reviews and updates security policies and procedures, and the security coordinator is trained in emergency response and risk management.*
