  formlarda tutarsızlık mevcut örnek olarak bir ek koyacağım seçilmesine rağmen alan boş olarak gelmekte.

Are you referring to the check box fields? If yes, I suggest you map them again because some of the options are mapped into its relevant options on the PDF.

You also have a few similar fields which I believe you're using with your show/hide conditions depending on the user's answer. If you want all of them to appear into a single field on the PDF, you need to place them into the same spot. Example for the following, one of the fields isn't on the your PDF. Once this is shown through condition an filled by the user, this will leave an empty field on the PDF.

I suggest you map those fields that gives empty result on the PDF and see how it goes.
