franciscoramosfotoI want to create a form that serves as a technical report after a bike inspection or service to give to our customers. I am a mechanic from a shop specializing in bicycles. I have all the checkpoints ready to be filled in the form. The form should include various components and tasks such as inspection, disassembly, lubrication, replacement, and other specific checks for bike parts like frame, motor, wheels, brakes, suspension, and more. I need the form to be created possibly from a PDF or a Numbers or Excel file. Additionally, I require that the amounts in the column labeled "valor servicio repuestos" (service and parts value) be summed up and displayed in the totals section. The sum should include VAT (IVA). This functionality is important for accurate billing and reporting. When asked if I want to create a ticket, I confirmed that I do. -
Christine Jotform SupportHi Francisco,
Thanks for reaching out to Jotform Support. We don't provide custom form creation or designing services, but you're welcome to work with a third party who may be able to help you. You can check out our Jotform Solutions Partner Directory for a list of consultants who've asked to be included.
You can get in touch with the right Solutions Partner for you by sorting them by location, type of service, industry, language support, or partner tier. Just fill out the contact form for the Jotform partner you'd like to work with, so they can make the necessary preparations to help you.
Keep in mind, though, that these parties operate independently of Jotform, so we assume no responsibility or liability for their work or their actions.
If you want to create a form using a PDF file, creating a form by uploading a PDF is easy. Let me show you how to do it:
- On your Workspace page, click on Create on the top-left side of the page.
- Select Form from the assets, and then click on Smart PDF Form.
- In Smart PDF Forms, click on Upload Document, and then select the PDF you want to convert to a form, or just drag and drop the PDF into the white upload area.
Sometimes the fields automatically created in the PDF file end up in the wrong place. If that happens, you’ll need to move and resize them. Check out the screencast below to see how it’s done: If some fields aren’t added automatically, you can manually add them to your form, and then match it to the PDF on the right side of the page. That's it. Once you set everything up, when the form is filled, it will automatically fill your PDF file. Check out the screencast below to see how it'll work: Give it a try and let us know if you need any help.
