Anclar un formulario con otro

Hola, lo que sucede es que quiero contratar el plan de pago pero tengo un problema, quisiera saber si se puede anclar datos desde un formulario con otro formulario.

Un ejemplo: primero crear un formulario para captar clientes dentro de eso tiene su nombre, cedula de identidad, etc..

en el otro formulario es para que el paciente se haga un informe clinico.

entonces mi duda es si esque se puede anclar a traves, por ejemplo, desde la cédula de identidad los dos formularios?
