Como vincular as respostas ao teste

Criei um teste de temperamento.

Nesse teste, para a pessoa acessar a resposta ela deve preencher o email ao final. O resultado dependerá das respostas que ela preencher.

Como faço pra vincular o resultado às respostas que a pessoa der?

E como faço pra mostrar pro sistema a resposta certa que deve aparecer de acordo com as alternativas que ela escolheu?
