Account Suspended: Collecting password on a form field

Identifiquei que não posso criar forms solicitando acessos. Login e senha.
Eu criei um formulário assim antes de descobrir que não podia.

Minha conta foi suspensa, pode excluir o formulário que estava criando com a solicitação de informações. e liberar a minha conta.

Now, let me help you with your question. I checked the Clone of Briefing Gestão de Tráfego Pago form which has violated our terms of use and I see that one of the form field is collecting password from the users.

I have deleted the offending field and reactivated your account. You can now log in to Jotform and get back to the My Forms page.
