Preciso criar um formulário para cadastro de clientes

Preciso de um formulário que coletará informações sobre dados de clientes (dados de identificação) com a devida autorização de LGPD, bem como dados de capacidade de pagamento e imóveis do mesmo.Esse cliente será abordado por um de nossos consultores e ele coletará junto ao cliente tais informações, esses dados devem ficar vinculados ao consultor específico (necessitamos aqui alguma forma de autenticação do consultor), somente ele (consultor) e o cliente podem alterar os dados.A idéia é que a cada visita do consultor ao cliente ele consiga coletar mais informações e vá enriquecendo o cadastro, para tal é necessária uma forma de busca de "fichas de clientes" cadastradas e vinculadas ao consultor.

no caso de um cliente ja estar cadastrado e não pertencer ao consultor a informação de quem é o responsável pelo cliente deve ser informada.

Com Jotform conseguirei fazer isso, qual a licença deverei contratar ?

For your consultant, you can set up an approval flow. Let me show you how:

1. In Form Builder, click on the Downward Arrow icon next to Form Builder at the top left corner of the page, and click on Approvals.
2. This will automatically create an approval flow for your form and you can start adding the elements you want to your Approval.

For them to be able to edit the submission, you can add an edit link to your Autoresponder Email. Let me show you how:

1. Open your form in Form Builder, and in the orange navigation bar at the top of the page, click on Settings.
2. In the Settings menu, click on Emails and hover your mouse over the Autoresponder Email.
3. Click on the Pencil icon, and in the window that opens, click on the Form Fields dropdown menu and select Edit Link.
4. After adding the link, click on Save, and you're done.
