To clarify, you want to add more than one element on each page of the card form, is that right? In a card form layout, you can only do that by using the Multi-line Question element. Let me walk you through it:

1. In the Form Builder, click on the Add Form Element on the left side of the page.
2. Select Multi-line Question under the Basic Elements.
3. Click the +Add a new field element. Then select the elements that can be added from the selection.
4. To edit the added element, click on the Edit icon.
5. From the Multi-line Question Properties, go to the added element and click on the Gear icon. You can enter your question in the Sublabel box.
6. Click on the Save button at the bottom of the page once you're done setting up the added form field.

• You can also set a certain format for the field from Validation. In my example, I set the field as numeric.

To make the fields side by side:

• Click on the + icon on the right side of the first added field.
