Account suspended due to password field

Olá, boa noite, venho por meio dessa mensagem tentar entender o porquê a minha conta foi suspensa trabalho fazendo vários serviços on-line tanto que sim faróis formulários pra instruções que trabalham na área é não tô entendendo o porque a minha conta foi suspensa, pesso encarecidamente para que eu vocês possam desbloquear a minha conta para que eu possa a volta a trabalhar e desde já pesso desculpa de quebrei alguma regra da plataforma, essa nunca foi a minha intenção

I checked your account and see that it was suspended because of the Academia de Polícia Civil Divisão de Recrutamento e Seleção form. Please note that collecting passwords with your form is against Jotform's Terms of use.

As your form has not received any submissions yet, I've gone ahead and removed the offending field from your form. Your account has been restored and you may log in to access your form again.
