Criamos dois formulários, um conseguimos testar e recebemos as respostas, depois criamos outro e nesse não recebemos quando alguém responde.

As checked, the submit button on that form is hidden unless the user clicked on the Section Collapse header above it.

So maybe your users were not able to find the submit button, though I can see there's one submission already.
