Now, coming back to your question, I cloned and checked your form. The issue happens because the Appointment field is placed under a Section Collapse element. Currently, it appears there is an unexpected behavior with the Section Collapse element; when it is conditionally shown, it does not automatically show the Appointment field inside it.

To fix this right away, you just need to add a new condition block to explicitly show the Appointment field whenever your specific locations are selected.
