Now, let me help you with your question. Can you tell what exactly you need some help with? If you need some help with creating your first web form, let me walk you through it:

1. On your My Forms page, click on Create Form in the upper-left corner.
2. Select from the four options that are shown, in my sample, I chose Start from Scratch.
3. You'll then see the layout options, which are Classic Form and Card Form. Choose one.
4. In Form Builder, click on Add Form Element in the upper-left corner to select elements, fields and widget to add to your form.
