BUG em formulário

Boa tarde, tudo bem?

Tenho um formulário pronto e gostaria apenas de mudar o modo de visualização das perguntas (deixar todas na mesma página), acontece que quando eu altero essa opção, o formulário em questão perde toda configuração e aparecem informações de outro formulário.

Conseguem me auxiliar, por favor?
