sobre ficha de anamnese

Gostaria de saber o por que na minha ficha de anamnese quando mando para a cliente ela preenche todos os campos, e ao finalizar não aparece a opção de enviar a ficha?

After investigating it a bit, I found a solution. It's really easy to fix. Let me show you how:

• In Form Builder, in the orange navigation bar at the top of the screen, click on Build.
• Click on the Add Form Element menu on the left side of the page.
• Scroll down, and drag and drop a Submit button to your form.
