Now, let me help you with your question. I made a test and was able to replicate the issue on my end. Currently, there are a lot of fields and 110 conditions on your form. Please note that the bigger the form is with lots of fields and conditions, most likely the form will become sluggish and will take a speed penalty when loading the form. I would suggest you divide your form to avoid this. Having a well-designed, shorter, dynamic, and easily used form is still the best way to make your form effective and have an excellent user experience.
