    Porém nada do que me relataram, estava sobre o formulário que eu havia feito, pois não pedi nada sobre dados sensíveis. Solicito então o desbloqueio da minha conta.

Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.

    Now, coming back to your question, I checked your account and found that it is currently suspended because of the Username & Password fields you have added to the form. Please note that it is not possible to use an authentication form in Jotform and creating such a form violates our terms of use.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar to an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected. To reactivate your account, we need your permission to delete the offending fields.
