أنا أريد تصميم نموذج اختيار درجات لطالب ثانوي وفق المنهج السعودي. على أن يكون من باب المحوسب الورقي معة سؤال. حاولت عدة مرات ولم أتمكن من تنفيذ النموذج بشكل صحيح. عندما حاولت تفعيل النموذج ظهرت لي مشكلة حيث لم أتمكن من تفعيل السؤال في نموذج الفضي. أريد أن أستعمل هذا النموذج في صياغة معة سؤال مثله مع المحافظة على التنوع وإعطاء نسق صعوبة الأسئلة. علينا الالتزام بالنصق الثنائي للكلمات والترتيب الجديد والمنسق للسؤال بودو.

Now, coming back to your question, if you want to create a form with multiple choice element, you can do it easily. Let me show you how:

1. In Form Builder, click on the Add Element on the left side of the page.

2. Scroll down and click on the Multiple choice element in the menu on the left, and add it to your form.

If you want to create a quiz form and show different results based on their answers, you can do that easily. You can do this by using Multiple choice/Single choice field, Calculation value, Form Calculation widget, and Conditional Logic. Let me show you how:

Set up Questions and Calculation values First, :

1. In Form Builder, click on Add Element on the left.

2. Click on the Multiple choice/Single choice element in the menu on the left and add it to your form as per your requirements.

3. Go to the Widgets tab, click on the Form Calculation widget, and add it to your form.

4. Click on the Gear icon beside the Multiple choice/Single choice element.

5. Click on the Options tab in the menu on the right and scroll down to the Calculation Values field.

6. Now, toggle Calculation Values to the ON position and set values for each answer. You need to repeat this process for every question.

Now, calculate the total value:

7. Click on the Wand icon beside the Form Calculation widget.

8. Click on the Add Field button in the menu on the right and add the questions field with a + sign to get the total value of the questions.

9. Finally, click on the Save button below.

Let's set the Thank you page to show the result based on the answer:

10. In Form Builder, click on the Settings tab in the orange navigation bar at the top.

11. Click on the Conditions tab in the menu on the left and select Change Thank you page condition from the list.

12. Now, set up your condition and click on the Save button below.

Also, you can show the Total score on your Thank you page. Let me show you how:

13. In the Thank you page condition, click on the Form Fields icon in the toolbar at the top.

14. Select the Total field from the list and add it to your Thank you page. It will display the result of the filler.

Also, you can easily Turn Your Form Into a Quiz or Exam where responders can see their score at the end. Let me show you how:

1. Go to the Quiz Form page, and click on the Open Quiz Form button.

2. In the window that pops up, click on Allow, select the form you want to turn into a quiz, and then click on the Continue button in the top-right corner of the Form Picker window.

3. Click on the Cross icon to close Form Picker, then in the Options section, click on Checkbox next to Email Results to and enter the email address and subject.

4. Then, click on Checkbox next to the Show Results to User After Submission option.

5. Now, scroll down, select the correct answers for your quiz, and click on Save Quiz at the bottom left of the page and that's it.

Now, when someone finishes your quiz and submits the form, they will see their results. Check out an example below to see how it'll look:

If you want to change the email content, check the Use Custom Email Page box and customize it as you like.
