Tengo un problema y es que de repente se ha deshabilitado un formulario que tenia creado, es muy importante para mi ya que forma parte un flujo de creacion de usuarios.
Me indica Login Credentials Collections
se llama Alta/Modificacion de Personal (Credenciales)
Me podríais ayudar a habilitarlo.

Now, let me help you with your question. I checked your form, and found that it's disabled due to collecting of Login Credentials (Password). Online Form Builders, including Jotform, are sometimes being used for identity theft, and we go to great lengths to prevent this. That is why collecting these types of data is considered a phishing activity, as stated in our Terms of Use.
To prevent your form from being flagged by our phishing detector, you should also refrain from using "password" as your label. Change it to something similar, like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.
To re-enable your form, you need to confirm with us that we can remove these offending fields.
Once we hear back from you, we'll be able to move forward with a solution. -
