    Now, coming back to your question, your account was suspended because this form 241126764938666 is collecting Credit Card details and a photo of the card. Please note that collecting Credit Card information is strictly prohibited as stated in our Terms of Use. I've deleted the offending fields and reactivated your account. You should be able to log back in now.

    Thanks for the information, but I think there is a confusion, this form that I am generating is from my restaurant, I am creating a customer loyalty card, (it is not a credit card), when I indicate enter the card number corresponds to the code that we have created for security, and the photo is the photo of the loyalty card, attached you can find the example of the card .

    Thanks for getting back to us. I understand that you are not trying to collect credit card details. You may a new field to collect the loyalty card number. Please use different words to make sure the fields will not be flagged for phishing again.
