Now, coming back to your question, when I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results:
It looks like you have not added your Google Maps API Key to the widget settings. Go here to get your Google Maps API Key. Then enter it in the widget settings. Let me show you how:
- In Form Builder, click on the widget and click on the wand icon on the right side.
- Enter the API Key in Google Maps API Key.
- Finally, click on Update below it.
