Now, coming back to your question, When I cloned your form, I had the same issue. It seems like you updated the form, but the PDF document was not updated. You can create the PDF file in a new tab again. The new file will include all the fields on the current form automatically. Let me show you how to fix it:
- In PDF Editor, click on the New PDF button.
- In the windows that opens, choose Use Default Layout option and that’s it.
