Now, coming back to your question, can you send me the link to your form so I can take a look at it for you? Let me show you how to do it:

1. In Form Builder, in the orange navigation bar at the top of the screen, click on Publish.
2. Click on Quick Share on the left side of the screen.
3. Click on Copy link under Share with link to copy your URL.

We've got a guide about Where to Find Your Form URL that will help you do it. Additionally, Can you capture a video of what’s going on and share it? Loom works great, or you can upload it to Google Drive or Dropbox.
