Now, coming back to your question, the issue is caused by the fact that you've added an unique field to your form. This prevents the same value from being submitted more than once.

If you'd like to disable it, follow the steps below:

1- Click on the settings button

2- In the settings screen, click on the "SHOW MORE OPTIONS" button and scroll down until you see the unique field section

3- Change this field to "No check"

Remember, if you do this, that field will no longer be unique and the same value will be allowed to be submitted multiple times.
