Regarding your issue with changing the title when sharing your form by link, please note that the Page Title (the text shown on the browser tab and link previews) is different from the form name and needs to be updated separately.
Here’s how to change the Page Title of your form:
- Open your form in Form Builder.
- Click on Settings in the top orange navigation bar.
- Go to Form Settings and click on Show More Options at the bottom.
- Scroll down to the Page Title section and enter your desired title.
- Click outside the input box to save your changes.
This will update the title that appears when sharing the form link, in browser tabs, and in social media previews.
If you want to customize the URL itself (the link), you can create a custom URL slug under the Publish tab by clicking the pencil icon next to the form URL and entering your preferred slug.

This happens because the form's "Page Title" (which shows up in the browser tab) is separate from the form's name. Renaming your form only updates its name inside your Jotform dashboard, not the browser tab title.
To fix this and remove "clone of" from the browser tab:
- Open your form in the Form Builder.
- Click on Settings in the orange navigation bar.
- Under Form Settings, click Show More Options.
- Scroll down to the Page Title section.
- Enter your desired title (without "clone of").
- Click outside the box to save.
