This happens because the form's "Page Title" (which shows up in the browser tab) is separate from the form's name. Renaming your form only updates its name inside your Jotform dashboard, not the browser tab title.

To fix this and remove "clone of" from the browser tab:

1. Open your form in the Form Builder.

3. Under Form Settings, click Show More Options.
4. Scroll down to the Page Title section.
5. Enter your desired title (without "clone of").
6. Click outside the box to save.
