PRoblemas con Formulario

Tengo un formulario despachador (https://form.jotform.com/253498028441057) el mismo tiene opciones de condicion para pasar a otro formulario segun URL, tengo el caso puntual de la opcion "ULTRASONIDO" (https://form.jotform.com/260356110707651) donde debe de pasar a dicho formulario y establecer el codigo de seguridad,

la condicion es: https://form.jotform.com/260356110707651?certificadout={certificado}

la pagina de ultrasonido esta bien

al hacer click me envia a la pagina :Clone of Certificacion HPU (Unidad acumulador)

podrian indicarme cual es el problema?
