How to add a title to the Fix Format widget?

En el demo del Widget de formato fijo veo que añade títulos antes de la caja de formato. Pero en el widget no dice como hacerlo...

• In the Form Builder, click on the widget's gear icon.
• In the Advanced tab, enter your preferred text to the Field label.
