    Now, let me help you with your question. I cloned your form to test it, and I easily replicated the issue. There is an easy fix for this, we just need to turn on the PDF section in Email Notification. Let me show you how:

    1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.

    2. Click on Emails on the left side of your screen.

    3. Hover your mouse on the right side of the email notifications.

    4. Click on Advanced.

    5. Turn on the Attachment PDF.
