    A minha conta foi bloqueada porem não sei o que fiz de errado, poderiam me esclarecer para que eu possa corrigir ?

I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.

    Now, let me help you with your question. I'm sorry that this is happening. I checked your account and saw that your account is suspended due to the system detecting phishing fields in your Formulário form. Please review Jotform's Terms of Use, also, to prevent this from happening again please don't collect login credentials. I've gone ahead and deleted the offending fields and reactivated the account.

I reviewed the account associated with the email address d****ft****a@g****.com and found that it has been suspended again due to your forms Formulário and CHECK LIST DE INFORMAÇÕES força tarefa containing a copyrighted logo. We understand this might not be the case with your forms, yet we had to act according to the general rule to comply with our Terms. To reactivate your account, we will need to either delete and purge the offending forms, or you can provide the following information:

    1. The URL where your form(s) are used
    2. Provide any documentation that will support your claims.
    3. Your website or proof of affiliation to legitimate institutions.

    Send this information to reviewers@jotform.com with Thread #13361181 as the subject line. We’ll let you know the status of your account after we’ve done a thorough review of everything.
