Speadsheet to Form: How to populate data?

Estou criando um formulário de recebimento de encomendas.

Ja tenho o cadastro das salas com os proprietários ou inquilinos. Quero que o atendente ao receber a encomenda so escolha o número da sala e automaticamente será preenchido o nome e email do interessado.

É possível fazer isso?

Now its working ok, only thing, the picture ddi not show when opened. (why?)
